Telemedicine Nurse Coordinator and Clinical Informatics Specialist - REMOTE

What You’ll Be Doing:

The Telemedicine Nurse Coordinator and Clinical Informatics Specialist is a dynamic position. ID Connect is a growing telemedicine company and the successful candidate for this position will be willing to adapt to new challenges. The candidate will perform a clinical and administrative role responsible for supporting patients receiving outpatient parenteral antimicrobial therapy (OPAT) and managing informatics, reporting, documentation, and billing activities for a busy telemedicine company.

The position has two areas of focus: telemedicine-based OPAT patient monitoring and clinical coordination, and clinical informatics, data reporting, database management, and billing support. The position may be filled by a qualified Registered Nurse (RN) or Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).

Essential Responsibilities

Telemedicine Patient Monitoring

Support the clinical operations of the organization's OPAT and Tele-Infectious Diseases (Tele-ID), and other telemedicine programs.
Conduct scheduled telephonic or virtual outreach to patients receiving outpatient intravenous antimicrobial therapy or other patient monitoring.
Monitor patient-reported symptoms, medication tolerance, vascular access concerns, laboratory completion, treatment adherence, and other indicators identified in the patient's care plan.
Review available clinical records, laboratory results, medication information, and physician recommendations within the nurse's authorized scope of practice.
Identify abnormal results, changes in condition, adverse medication reactions, missed doses, line complications, or other clinical concerns and escalate them promptly according to established protocols.
Assist patients and caregivers with questions regarding treatment plans, laboratory schedules, medication administration, vascular access care, follow-up appointments, and physician recommendations.
Coordinate communication among Tele-ID physicians, primary care providers, infusion pharmacies, home health agencies, laboratories, skilled nursing facilities, and other involved organizations.
Obtain and organize medical records, laboratory results, discharge documentation, medication lists, and other information necessary for ongoing OPAT monitoring.
Document patient contacts, clinical observations, interventions, escalations, and care coordination activities accurately and promptly in the designated electronic systems.
Assist with transitions of care, including hospital-to-home or hospital-to-facility transitions, and help ensure continuity of antimicrobial treatment.

Clinical Informatics, Reporting, and Billing

Maintain, validate, and analyze clinical, operational, quality, and billing-related data for OPAT and Telemedicine services.
Create and maintain reports, dashboards, worklists, patient registries, and performance-tracking tools using Microsoft Excel and database systems.
Use Excel functions such as formulas, filters, lookup functions, conditional formatting, pivot tables, data validation, charts, and data-cleaning tools.
Enter, extract, reconcile, and audit information from electronic health records, practice-management systems, billing platforms, and internal databases.
Track program metrics such as patient volume, monitoring activity, laboratory completion, adverse events, escalations, hospital readmissions, treatment completion, documentation timeliness, and outcomes.
Assist with charge capture, billing reconciliation, claim-support documentation, and identification of missing or inconsistent data.
Apply working knowledge of relevant ICD-10-CM, CPT, HCPCS, evaluation and management, remote monitoring, telehealth, and other applicable billing codes, as appropriate to the role.
Investigate discrepancies among clinical documentation, service records, charges, and billing reports.
Help define data standards, reporting requirements, workflows, and business rules for OPAT and Telemedicine operations.
Protect data integrity by identifying duplicate, incomplete, inaccurate, or inconsistent records and coordinating corrections.
Participate in system testing, workflow development, database improvements, and implementation of new clinical or reporting tools.

Collaboration and Organizational Responsibilities

Collaborate with Telemedicine physicians, credentialing managers, clinical supervisors, team leaders, call center agents, and informatics personnel.
Perform duties in a manner consistent with the organization's service-management philosophy and standards for interactions with patients, caregivers, physicians, peers, staff, and other departments.
Identify, develop, and participate in process-improvement opportunities that enhance quality, patient safety, operational efficiency, data accuracy, and cost-effectiveness.
Participate in quality-improvement projects and assist with collecting, analyzing, and presenting program data.
Maintain a strong focus on customer service and perform all duties in a manner that promotes optimal patient, provider, and internal customer satisfaction.
Attend required compliance, infectious diseases educational lectures, and organizational training.
Perform other duties consistent with the position and applicable professional scope of practice.

Required Qualifications

Current, active, and unrestricted RN or LPN/LVN license.
Graduation from an accredited nursing program.
Clinical nursing experience in ambulatory care, infectious diseases, infusion therapy, home health, care coordination, telehealth, medical-surgical nursing, or a related setting.
Proficiency with Microsoft Excel, including formulas, filters, sorting, lookup functions, pivot tables, and basic data analysis.
Experience using electronic health records, clinical databases, practice-management systems, or similar healthcare information systems.
Working knowledge of medical terminology, clinical documentation, and healthcare billing processes.
Strong attention to detail, organization, analytical thinking, and problem-solving skills.
Strong written, verbal, and interpersonal communication skills.
Ability to manage clinical priorities, reporting deadlines, and multiple workstreams in a remote environment.

Ability to maintain patient confidentiality and comply with HIPAA and information-security requirements.

Preferred Qualifications

Bachelor's degree in nursing, health informatics, healthcare administration, data analytics, or a related field.
Two or more years of relevant nursing experience.
Experience supporting an OPAT, infectious diseases, antimicrobial stewardship, infusion, or Telemedicine program.
Experience with remote patient monitoring or chronic care management workflows.
Advanced Excel skills, including Power Query, complex formulas, macros, or dashboard development.
Experience with relational databases, SQL, data visualization tools, or business-intelligence platforms.
Experience with coding audits, billing reconciliation, utilization reporting, or revenue-cycle operations.
Experience participating in quality-improvement, patient-safety, or clinical workflow redesign initiatives.
The successful candidate may have interest in account management, customer success or other possible customer-facing roles for future role expansion.

Working Conditions

Work is primarily performed in a remote, telemedicine setting.
Requires extended use of a computer, telephone, electronic health record, and video-conferencing systems.
May require communication with patients and clinical partners across different locations and time zones.
The employee must maintain a private, secure workspace when handling protected health information remotely.
Working hours are generally 8:00 AM–5:00 PM ET.

Reporting Relationship

This position reports to the VP of Practice and Services Implementation and works closely with Tele-ID physicians, OPAT RN Coordinator, and informatics teams.
